Rachel will ask what brought you to the appointment, when symptoms began, how they have changed, and how they affect work, relationships, sleep, concentration, or other parts of daily life.

2. Review relevant history

The evaluation may include medical history, previous mental health care, current and past medications, substance use when clinically relevant, and family mental health history.

3. Assess current mental functioning

The clinical interview may include observations of mood, affect, speech, thought processes, attention, cognition, insight, judgment, and other aspects of mental functioning when relevant.

4. Discuss appropriate next steps

After reviewing the available information, Rachel may discuss a clinical impression and options such as therapeutic support, medication management, further assessment, medical follow-up, or referral when appropriate.
